What to expect during your 1st visit with Debra Harrigan, ND (Aus):
Prior to your first appointment we will email you our Intake Packet which includes an Initial Consultation Form and a Diet Diary Template
Please complete these forms and email them back to us 72-hours prior to your first appointment.
In addition to the intake forms please have available for your first appointment a copy of all lab results ordered during the previous 12-months as well a copy of any imaging reports (x-ray, MRI, CAT scan, etc) from the previous 12-months. Please inform us if your Physician's office requires a records release form to be sent from our office. You can also request that your Physician's office fax your records directly to our office on: 972 385-7980.
You will also need to bring with you to your first appointment the bottles (empty or full) of any pharmaceutical medication and/or any Nutritional Supplements taken during the previous 6-months.
Please allow 90 minutes for your first appointment.
The cost for your Initial Consulation will be $150.00, which can be paid by cash, check or credit card.
Payment Policy
We accept cash, check, or Credit Card. We do ask for payment at the time that service is provided unless previous arrangements have been made.
Confidentiality
Patient confidentiality is maintained at all times. No information is given to anyone outside of the office unless it is at your request.
Cancellation Policy
To help keep our office running smoothly, and to allow us to provide sufficient time to all clients, we ask that cancellations be made at least 24 hours before your scheduled appointment time.
